Job summary

Cap City Supported Living provides support services for young adults and adults with developmental disabilities, offering socialization, community-based programs, and home personal care. The role involves direct client care under the ISP and ensuring safety, health, and daily activities are supported in residential settings.

The ideal candidate has a high school diploma, a valid driver’s license, and required background checks.

Qualifications

• High School Diploma or equivalent.
• Valid Driver’s License (no more than 6 points).
• Current Proof of auto insurance (state minimum).
• 18 years or older.
• FBI and/or BCI Background Check (5123:2-1-05.1 ORC).
• CPR and First Aid Training (5123:2-2-.1 ORC).
• 8hr DODD Training.
• Medication Administration Training.

Responsibilities

• Provide direct care to client’s based upon their needs as well as the client’s ISP.
• Coach/assist client with routine tasks, housekeeping, meal prep, grocery shopping, and personal care.
• Coach/assist clients in safety by performing drills and recognizing emergencies.
• Assist client with day to day finances and access to community activities.
• Monitor medical needs: scheduling, appointments, medications or administration of health activities.
• Provide safe transportation in regards to social, medical and general needs.
• Complete documentation accurately per agency, county and client requirements.
• Comply with OSHA, HIPAA, rights of clients, and related regulations.
• Advocate to protect and advance client rights.
• Embrace Cap City mission, vision and policies.
• Communicate with supervisors and peers on health and safety needs.
• Other duties as assigned.

Cap City Supported Living provides support services geared toward young adults and adults who live with developmental disabilities. Our clients benefit from socialization and community- bases programs, and Home Personal Care.

Essential Job Functions:

• Provide direct care to client’s based upon their needs as well as the client’s Individual Services Plan (ISP).
• Coach/assist client with accomplishing routine tasks, housekeeping, meal prep, grocery shopping, and personal care.
• Coach/assist clients in the area of safety by performing routine fire and tornado drills, recognizing emergencies as well as responding as needed.
• Coach/assist client with day to day finances.
• Assist client with access to available community activities, exercise, and maintaining relationships.
• Assist clients with monitoring medical needs including scheduling, attending medical appointments, filling prescriptions, and self or delegated administration of medications or health related activities.
• Provides or facilitates safe transportation in regards to social, medical and general needs.
• Attend all meetings in regards to clients, employee requirements, and or agency needs.
• Complete all documentation accurately and services according to the agency, county and client requirements.
• Complies with OSHA, HIPAA, Rights of Clients (5123.62 ORC), Clients Options Waiver/HPC (5123:2-13-04-OAC)
• Advocating to protect and advance the rights of clients
• Embrace and embody the mission, vision and policies and procedures of Cap City.
• Communicate with supervisors, peers on any medical, health and safety needs.
• Other duties as assigned.

Physical Demands:

• Requires mobility and physical activity: Having adequate range of motion and mobility to work in an office, residentials, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended period of time, bending, twisting, reaching, balancing, and occasionally lifting and carrying up to 30 pounds.
• Requires the ability to defend oneself and clients in physically aggressive situations through the use of approved descalation techniques.
• Requires communication, expressing or exchanging ideas by means of spoken and written word. Talking is required to share information regarding clients.
• Requires hearing: Hearing is required to receive and communicate detailed information thought oral communication and ensure ability to supervise clients.
• Requires seeing: Clarity of vision at 20 inches or less and at a distance to complete paperwork, observe and supervise client.

Mental Demands:

• Requires the ability to reason and be empathetic to clients and teammates.
• Requires the ability to cooperate with clients, teammates, supervisors and outside supports for clients to exchange ideas, information and instruction.
• Requires the ability to work under stress and emotionally changed situations.
