Job summary

Cap City Home Care Services is seeking caregivers to provide direct care in residential settings, assist with daily activities, meal prep, shopping, and personal care. The role includes helping with finances, community activities, and monitoring medical needs such as appointments and medications.

Ideal candidates hold a High School Diploma or equivalent, a valid driver’s license with good standing, and CPR/First Aid training.

Qualifications

• High School Diploma or equivalent required.
• Must have valid driver’s license (no more than 6 points).
• Current proof of auto insurance (state minimum).
• Must be 18 years or older.
• FBI and/or BCI background check.
• CPR and First Aid Training.
• 8hr DODD Training.
• Medication Administration Training.
• Any other trainings deemed necessary by Cap City Home Care Services.

Responsibilities

• Provide direct care to clients based on ISP and needs.
• Coach/assist with routine tasks, housekeeping, meal prep, groceries, and personal care.
• Coach/assist in safety through drills and emergency recognition/response.
• Assist clients with day-to-day finances and accessing community activities.
• Monitor medical needs—appointments, prescriptions, and medications.
• Provide safe transportation for social, medical, and general needs.
• Attend meetings and complete documentation per agency and client requirements.
• Comply with OSHA, HIPAA, and relevant client rights and policies.

Job description

Essential Job Functions:

• Provide direct care to clients based upon their needs as well as the clients Individual Services Plan (ISP).
• Coach/assist client with accomplishing routine tasks, housekeeping, meal prep, grocery shopping, and personal care.
• Coach/assist clients in the area of safety by performing routine fire and tornado drills, recognizing emergencies as well as responding as needed.
• Coach/assist client with day to day finances.
• Assist client with access to available community activities, exercise, and maintaining relationships.
• Assist clients with monitoring medical needs including scheduling, attending medical appointments, filling prescriptions, and self or delegated administration of medications or health related activities.
• Provides or facilitates safe transportation in regards to social, medical and general needs.
• Attend all meetings in regards to clients, employee requirements, and or agency needs.
• Complete all documentation accurately and services according to the agency, county and client requirements.
• Complies with OSHA, HIPAA, Rights of Clients (5123.62 ORC), Clients Options Waiver/HPC (5123:2-13-04-OAC)
• Advocating to protect and advance the rights of clients
• Embrace and embody the mission, vision and policies and procedures of Cap City.
• Communicate with supervisors, peers on any medical, health and safety needs.
• Other duties as assigned.

Physical Demands:

• Requires mobility and physical activity: Having adequate range of motion and mobility to work in an office, residentials, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended period of time, bending, twisting, reaching, balancing, and occasionally lifting and carrying up to 30 pounds.
• Requires the ability to defend oneself and clients in physically aggressive situations through the use of approved descalation techniques.
• Requires communication, expressing or exchanging ideas by means of spoken and written word. Talking is required to share information regarding clients.
• Requires hearing: Hearing is required to receive and communicate detailed information thought oral communication and ensure ability to supervise clients.
• Requires seeing: Clarity of vision at 20 inches or less and at a distance to complete paperwork, observe and supervise client.

Mental Demands:

• Requires the ability to reason and be empathetic to clients and teammates.
• Requires the ability to cooperate with clients, teammates, supervisors and outside supports for clients to exchange ideas, information and instruction.
