Job summary

Easterseals PORT Health in Smithfield, NC is seeking a Direct Support Professional to join our Individual and Community Services team. This part-time role supports individuals in their homes and local communities, helping them build independence and achieve personal goals.

You will document daily activities, coordinate with families, and contribute to meaningful, compassionate care for people of all ages and abilities. Local travel is expected as part of the role.

Qualifications

• High school diploma or equivalent required.
• Valid North Carolina Driver’s license, reliable transportation and vehicle insurance required.
• Experience working with individuals with disabilities preferred, but not required.
• Interpersonal skills, attention to detail, and dependability are a must.

Responsibilities

• Support the consumer with personal care needs and companionship.
• Document daily activities and goals met in an electronic format.
• Coordinate and collaborate with the family of the consumer to work on goals that promote independence, socialization and overall personal development.
