LifeScape is seeking a Direct Support Professional to assist adults with disabilities in living as independently as possible. This role emphasizes movement, problem-solving, teamwork, and meaningful human connection.

You will follow Individual Service Plans, support daily activities, assist with personal care, transportation, and appointments, and help build skills for independence in a safe, respectful environment.

Qualifications

• Willingness to learn and complete required training.

Responsibilities

• Support individuals with daily living activities and independence.
• Follow ISP and help people reach personal goals.
• Assist with personal care, meals, hygiene, and transfers.
• Support community involvement, recreation, work activities, and transportation.
• Provide medication support as trained and authorized.
• Attend medical appointments and communicate changes to the care team.
• Maintain accurate documentation and reports.
• Create a safe, positive environment each shift.
