Zusammenfassung

CCAR Industries, Locust Street Day Program, seeks a full-time Developmental Trainer/Direct Support Person to support individuals in activities of their choice at the program site or in the community.

The position runs Monday–Friday, 7:00 am to 3:30 pm (40 hours/week) with weekends and major holidays off. Starting pay is $18.25 per hour, with increases after DSP and training completion and after medication administration training.

Qualifikationen

• Must possess a valid driver's license with insurability requirements.
• Must be at least 18 years old.
• Must provide proof of a high school diploma (via transcripts) or proof of a GED.
• Must pass a criminal background check as required by law.
• CPR/First Aid certification (American Heart Association or American Red Cross).
• Complete DSP training per IDHS requirements.
• Pass the S-TOFHLA health literacy test.
• Complete medication administration training and recertifications.

• Complete vehicle safety checks and maintain vehicles as assigned.
• Communicate with coworkers about location/program and needs.
• Be prepared to work at any program or location with any served individual.
• Other duties as needed.

CCAR Industries is searching for a full-time, 40 hour per week direct support person to work at our Locust Street Day Program. The Developmental Trainer/Direct Support Person (DSP) will support the people served as they engage in activities of their choice at the program site or in the community.

SCHEDULE

This is a permanent full-time, daytime position on Monday - Friday from 7 am to 3:30 pm (40 hours per week). Weekends and major holidays off.

ESSENTIAL REQUIREMENTS

• Must possess a valid driver's license with a driving record that meets the agency's insurability requirements (Employees are required to transport persons served to community activities of their choosing, on medical visits as they arise, and to/from the program when needed.)
  
• Must be at least 18 years old
• Must provide proof of a high school diploma (via transcripts) or proof of a GED from an accredited school
• Must pass a criminal background check, as required by the Healthcare Worker Background Check Act (225 ILCS 46) and a DCFS Child Abuse and Neglect background check
• Successfully complete and maintain American Heart Association or American Red Cross CPR/First Aid certification
• Successfully complete Direct Support Person training in accordance with IDHS requirements
• Pass the Short Test of Functional Health Literacy in Adults (S-TOFHLA)
• Successfully complete medication administration training and annual recertifications to maintain medication administration status

DUTIES

• Develop a rapport with persons served
• Support individuals in self-advocacy and making informed choices
• Assist in planning and implementing day program activities based on the input of those served
• Transport people to activities of their choosing and to/from the program to their homes in agency-owned vehicles
• Support people by assisting with daily living needs, such as grooming, preparing food, and restroom hygiene
• Assist with setting-up and cleaning-up before/after breaktimes
• Provide support and guidance regarding workplace behavior/etiquette
• Review individualized plans and document the services/supports provided on a daily basis
• Accurately record arrival, departure, and other activity times
• Actively participate in team meetings
• Assist in planning and implementing program-based or community activities based on the input of those served
• Assure that the workplace is clean, neat, and free of potential safety hazards
• Complete vehicle safety checks, fuel and maintain vehicles, as assigned
• Communicate with co-workers, supervisors, and other team members about location/program and person-specific needs
• Be prepared to work at any program or location with any of the people served by the agency
• Other duties as needed

WAGES

This position starts at $18.25/hour with a $0.50/hour increase when DSP and programmatic training is complete. An additional $0.25/hour wage increase will be given when all facets of medication administration training are complete.
