Lutheran Social Services of Illinois is hiring a part-time Direct Support Person for Mondays and Wednesdays. You will assist individuals with self-care, hygiene, meals, housekeeping, socialization and recreations, and you will collect, record, and file data as required.

Essential functions include goal support, service planning, transportation, and maintaining dependable attendance, all while upholding dignity and respect for clients and teammates.

Qualifications

• High school diploma or equivalent required.
• Background check clearance required.
• CPR and First Aid basics required.
• Must pass the TABE test.
• Must be 18 years of age or older.
• Available to work all shifts, including overnights and weekends.
• Clear written and verbal communication.
• Valid driver's license in good standing.
• Access to reliable transportation.
• Valid IL liability insurance coverage required.

Responsibilities

• Monitor, implement, and document individual personal goals and behavior programs.
• Participate in development of the individual Service Plan via the daily log, programming suggestions, and staffing.
• Participate in community integration, including providing transportation to the person(s) served.
• Maintain regular and consistent work attendance.
• Demonstrate professional, positive behavior and carry out responsibilities with integrity and respect.
