Job summary

Extended Family Incorporated is seeking a dedicated DSW to assist clients and families with in-home and facility-based care. You will perform personal care tasks, document daily activities, and follow ISP guidelines to ensure safety and comfort for clients across settings.

The role emphasizes reliability, empathy, and teamwork, with a focus on respectful interactions and adherence to state regulations. Training, transportation, and potential advancement opportunities are included.

Qualifications

• High School Diploma or equivalent required.
• Medical or in-home healthcare training preferred or DSW on-the-job experience.
• CPR & First Aid certifications a plus.

Responsibilities

• Provide the best care per the ISP for each client.
• Assist with in-home or facility-based personal care needs: bathing, dressing, grooming, feeding.
• Assist with exercises, toileting, positioning, and transfer/ambulation.
• Support with medication administration and routine self-care tasks.
• Document daily activities and report changes to supervisor promptly.
• Follow emergency procedures and state regulations; maintain confidentiality.
• Report abuse or neglect and comply with company policies.
• Perform other duties as assigned by Supervisor or Executive Director.
