Job summary

Centra Healthcare Solutions is seeking an experienced Speech Language Pathologist / SLP for a permanent/direct hire role in a SNF / LTC setting. The position requires SLP-CCC, current state licensure, and CPR certification, with an emphasis on interdisciplinary care and patient-centered outcomes.

Qualifications

• Must have graduated from an accredited school.
• Current state license as an SLP or in the process of licensure.
• Current CPR certification.

Responsibilities

• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills.
• Candidate must be an SLP-CCC.
