Qualifications

  • Complete a pre-employment / referral physical and a TB test per CDC guidelines.
  • Must be able to lift up to 35 lbs.
  • Must have a valid driver’s license and proof of auto insurance.

Responsibilities

  • Provide personal care including bathing, skin care, toileting, dressing, feeding and shaving as per the Service Plan.
  • Assist with transfers and ambulation; provide light housekeeping, meal prep, laundry, shopping, transportation and companionship.
  • Complete and maintain Direct Care Daily Activity Logs and observation reports; report changes to the Program Director.
  • Attend required training and in-service sessions; complete knowledge and competency tests annually.
  • Attend to fill-in assignments as scheduled by the Program Director or Placement Coordinator.
  • Respond promptly to calls from CHEER; adhere to scheduling changes as needed.

DIRECT CARE WORKER

SUMMARY DESCRIPTION

The primary focus of this position is to provide high-quality, efficient, and safe personal assistance and non-medically skilled service to customers consistent with an established Service Plan focusing on Activities of Daily Living.

ESSENTIALDUTIES &RESPONSIBILITIES
  • Provide services to customers by following the Service Plan, which may include the following types of care: personal care to include bathing, skin care, toileting, dressing, feeding, and shaving. Physical care to include assisting with transfers and ambulation. Supportive care to include preparing meals, light housekeeping, laundry, changing linens, shopping, local transportation, and companionship.
  • Successfully complete all required training requirements, including recurrent and ongoing requirements. Must successfully complete knowledge and competency tests at least annually, demonstrating understanding and ability to perform Personal Assistance Services duties.
  • Complete assigned work, as per the Service Plan, accurately and within the appropriate time.
  • Document on Direct Care Daily Activity Log and observation report form as indicated. Report problems or significant changes in customer condition to the Program Director immediately.
  • Complete activity logs accurately, completely, and within the required time.
  • Attend all in-service training sessions and meetings as required.
  • Accept fill-in assignments as schedule allows and per request of the Program Director and/or Placement Coordinator.
  • Respond to calls from CHEER immediately or as soon as possible, as changes in scheduling do occur.
  • Must complete the following requirements prior to assuming the position:
    -Complete a pre-employment / referral physical.
    -Complete a pre-employment Tuberculosis test consistent with the current procedure recommended by the Centers for Disease Control and Prevention.
  • Provide documentation of educational preparation and work history.
  • Must be able to lift up to a maximum of 35 lbs.
  • Must have a valid driver’s license and proof of auto insurance.
  • Contributes to team effort by performing other related duties as assigned or required.

This position reports to the PASA Program Director.

Based on customer referral locations within Sussex County, Delaware.

Regular Part-Time & Casual positions, Non-Exempt.
