Responsibilities

• Assist patients with personal care activities as prescribed, including hygiene, mobility, and elimination.
• Maintain a safe environment, using equipment properly.
• Practice Universal Infection Control Precautions.
• Assist with home maintenance activities like light housekeeping and meal prep.
• Monitor and record vital signs and body functions.
• Perform safe transfer, ambulation, and positioning techniques.
• Identify needs for intervention and report to RN or Clinical Director.
• Respect patient confidentiality and autonomy.
• Document care provided and responses, submitting reports timely.
• Work within your training and experience limits.
• Ensure compliance with laws, regulations, and policies.
• Manage caseloads efficiently and deliver cost-effective care.
• Meet or exceed service standards and conduct business ethically.
• Use supplies and equipment effectively.
• Complete at least 12 hours of in-service training annually.

Qualifications

Entry level; prior experience is not mandatory but a caring attitude and willingness to learn are essential.

Employment Details
