Job summary

Visiting Nurse Association Community Services is seeking a Home Health Aide (HHA)/Direct Care Worker to provide compassionate personal care to individuals in their own homes, following each client's care plan. Whether certified or with relevant skills, you will support daily activities, mobility, and safety, while building meaningful relationships and contributing to clients' independence.

Responsibilities

• Assist with personal care: bathing, grooming, dressing, feeding.
• Provide mobility support: ambulation, transfers, positioning.
• Support physical therapy plans with exercises under supervision.
• Monitor and report changes in physical or emotional health.
• Perform light housekeeping in client homes.
• Prepare meals and shop for groceries and supplies.
• Maintain a clean, safe home environment.
• Document care and submit timely reports.

Do you have a heart for helping others live safely and independently in their own homes? As a Home Health Aide (HHA) / Direct Care Worker, you'll provide compassionate personal care and support to individuals who rely on you daily.

What You'll Do:

Your responsibilities may include, but are not limited to:

• Assisting with personal care: bathing, toileting, grooming, dressing, feeding, oral hygiene, nail care, shaving
• Mobility support: ambulation, transferring, positioning clients in bed, using assistive devices
• Supporting physical therapy plans, passive and active exercises under supervision
• Monitoring and communicating changes in physical or emotional health
• Performing light housekeeping: laundry, tidying bedrooms, kitchens, and bathrooms
• Preparing meals (including special diets), shopping for groceries, and household supplies
• Maintaining a clean, safe, and comfortable living environment
• Using PPE (Personal Protective Equipment) when appropriate
• Documenting care and submitting timely reports via telephony system

Who We're Looking For: Required Qualifications:

• At least 18 years of age
• Completion of a formal certification program or ability to pass a skills/competency test
• Ability to read, write, and follow a written Plan of Care
• Good verbal, written, and interpersonal communication
• Neat appearance and good personal hygiene
• Dependability, reliability, and sound judgment
• Friendly, patient, and respectful with a sincere interest in caring for others
• Ability to lift up to 50 lbs and perform physical caregiving tasks
• Maintain confidentiality and comply with HIPAA regulations

Preferred Qualifications:

• Valid driver's license, insured vehicle in good repair, and clean driving record
• Ability to travel independently to clients' homes (unless exempt)
• Must be able to work at least two weekends per month as scheduled
