Responsibilities:

• Assist patients with personal care activities such as hygiene, mobility, and elimination as prescribed in the care plan.
• Report to the RN or Director of Clinical Services if unable to perform a task.
• Practice Universal Infection Control Precautions.
• Assist with home maintenance activities like light housekeeping and meal prep.
• Use safe transfer, ambulation, and positioning techniques.
• Respect patient confidentiality and autonomy.
• Document care provided and responses in a timely manner.
• Perform duties within your scope of training and experience.
• Ensure compliance with all applicable laws, regulations, and agency policies.
• Meet or exceed service standards and conduct business ethically and professionally.
• Adhere to the company's Code of Conduct and confidentiality policies, including HIPAA guidelines.
