Responsibilities:
- Assist patients with personal care activities such as hygiene, mobility, and elimination.
- Maintain a safe environment for patients, using equipment safely.
- Practice Universal Infection Control Precautions.
- Assist with home maintenance activities like light housekeeping and meal prep.
- Monitor and record patient vital signs and body functions.
- Practice safe transfer, ambulation, and positioning techniques.
- Identify the need for interventions and report to the RN or Clinical Director.
- Respect patient confidentiality and autonomy.
- Complete and submit documentation promptly.
- Ensure compliance with laws, regulations, and policies.
- Manage caseload efficiently and deliver cost-effective care.
- Meet or exceed service standards and conduct business ethically.
- Use supplies and equipment effectively.
- Complete at least 12 hours of in-service training annually.
- Maintain regular attendance and adhere to the Code of Conduct.
- Support the mission and vision of 365 Health Services.
- Follow orientation and confidentiality policies, including HIPAA guidelines.

Salary: $10.00 - $15.00/hr starting, plus OT for hours over 40/week
