Job summary

Communicare in Mississippi is seeking a dedicated care worker to provide direct care for individuals with ID/DD in our supervised living program.

You will support development of social, self-help, and daily living skills, plan meaningful activities, encourage independence, and assist with transportation.

A high school diploma or GED is required; CPI certification or ability to complete CPI training is needed; a valid driver’s license and auto insurance must be maintained.

Qualifications

• High school diploma or GED (required).
• Ability to perform responsibilities as listed.
• Must be CPI certified or able to complete CPI training and any DMH/Communicare required training.
• Ability to relate positively to individuals served and their families.
• Current valid driver’s license, auto insurance, and insurable driving record maintained; transportation to individuals served.

Responsibilities

• Provide direct care for individuals designated as ID/DD in supervised living program.
• Assist with acquisition, retention, or improvement of social self-help, socialization, and ADL skills (e.g., money management, medication management).
• Plan and facilitate meaningful daily activities to foster independence and personal choice.
• Support community integration activities.
• Monitor and supervise individuals at all times during service provision (in home, transport, or community).
• Serve as a member of the Communicare Treatment Team and participate in meetings.
• Perform other duties as assigned.
• Work under supervision of the Supervised Living Supervisor or another designated supervisor.
• Transport clients to activities, appointments, etc.

• Assist with acquisition, retention, or improvement in social self‑help, socialization, and other adaptive skills that enhance social development and proficiency in activities of daily living (e.g., money management, medication management).
• Plan and facilitate meaningful daily activities, including the acquisition and maintenance of skills, fostering positive social behavior and interpersonal competence, greater independence, and personal choice.
• Support community integration activities.
• Maintain responsibility for monitoring and supervising individuals at all times during service provision, whether in a supervised living home, during transport, or in the community.
• Serve as a member of the Communicare Treatment Team and participate in team meetings as required.
• Perform other duties as assigned.
• Work under the supervision of the Supervised Living Supervisor or another designated supervisor.
• Transport clients to various activities, appointments, etc.

Qualifications

• Ability to relate positively to individuals served through the Supervised Living program and their families.
• Current valid driver’s license, automobile insurance, and an insurable driving record must be maintained continuously throughout employment; must be able to provide transportation to individuals being served.
