Position Overview

The Direct Care Worker (DCW) provides personal care and related services in the home, under the supervision of the Clinical Director and/or appropriate supervisor. Scheduling is coordinated by the Client Services Coordinator and/or supervisor.

Essential Job Functions

• Assist clients with personal hygiene, grooming, and dressing
• Maintain a safe and healthy environment
• Plan and prepare nutritious meals and assist with feeding if needed
• Run errands as instructed
• Assist with transfers and ambulation
• Provide medication reminders
• Encourage independence according to the nursing care plan
• Engage clients in activities to promote mental alertness
• Offer emotional and psychological support, establishing trust with clients and families
• Maintain confidentiality
• Complete visit reports and incorporate them into clinical records
• Report changes in client condition or home situation
• Follow instructions from supervisors and report pertinent changes
• Perform routine housekeeping tasks as instructed
• Participate in in-service education
• Coordinate visits with supervisors weekly

Requirements

• High school diploma or equivalent preferred
• Basic math, reading, and writing skills
• Ability to speak, understand, read, and write English
• Completion of a basic aide training program and/or Direct Care Worker test as per state/federal requirements
• One year of healthcare-related experience preferred
• Reliable transportation
• Health screenings and background checks
• Valid driver’s license and acceptable MVR

Physical Requirements

• Ability to read 12-point font or larger
• Ability to hear and speak clearly
• Ability to stoop, bend, and lift up to 50 pounds
• Ability to carry up to 10 pounds upstairs

Work in various environments, potentially exposed to allergens and other conditions.
