Qualifications

• Driver’s license is required to visit patient homes.

Responsibilities

• Assist patients with daily activities (transfers, bathing, mobility).
• Perform light housekeeping and personal care tasks.
• Maintain accurate records of patient care and progress for supervisor review.
• Engage patients to promote mental alertness through conversation and activities.
• Administer medications and vaccines as directed by supervising clinicians.

Job Summary

We are looking for a Direct Care Worker to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
• Direct Care Workers may assist consumers with the following activities:
  a. Self-administration of medications for consumers who are competent to direct the
  care
  b. Housekeeping
  c. Personal care including grooming and dressing
  d. Eating and meal preparation
  e. Oral hygiene and denture care
  f. Toileting and toilet hygiene
  g. IADL assistance
  h. Administering emergency first aid
  i. Providing or arranging for social interaction
  j. Providing transportation

Qualifications

• High School Diploma or GED
• One year prior professional experience; but willing to train
• Driver’s license required
