Dietitian / Per Diem
Provides personal care and professional excellence by assessing nutrition status and addressing the nutrition needs of clients both inpatient and outpatient. Provides the appropriate nutrition education for patients. Works with physicians and staff to promote the best nutritional environment for clients.
Wood County Hospital is accepting applications for a per diem Clinical Dietitian. Primary job responsibilities will be clinical dietetics, providing nutritional assessment and re-assessment of inpatient clients. Will also be responsible for inpatient medical nutrition therapy. Specially, looking for assistance with weekend coverage and possible week days. May be cross-trained in other areas.
Job Duties
Inpatient and General Responsibilities
- Identify inpatient clients that need to be assessed.
- Able to properly enter orders for assessments, reassessments and the corresponding communication orders in the EMR.
- Properly assesses the nutrition status of individual patients.
- Develops nutrition care plans for inpatients determined to have a nutritional diagnosis using the Nutrition Care Process.
- Properly reviews diet orders for patients. Able to make allowable corrections per physician privileges to diet orders, add orders for supplements/snacks and corresponding communication orders.
- Accurately monitors and evaluates the on-going effects of nutritional intervention on individual patient nutritional status.
- Professionally plans, organizes, implements and evaluates nutritional education for patients; arranges for individual patient follow-up as needed.
- Documents appropriately in patient’s medical record after each patient contact/assessment/reassessment.
- Professionally communicates pertinent information to other health care professionals; discusses individual patient nutrition care needs with health team members; educates health team on nutrition-related topics.
Outpatient Responsibilities
- Able to correctly schedule client for appointment using Cerner Scheduling.
- Assess and provide medical nutrition therapy using the Nutrition Care Process per physician referral.
- Assess Initial Diabetes Education clients with Pre-Diabetes, Type 1, Type 2, and/or gestational diabetes.
- Develop plan of care for diabetes education clients, collaborating with other members of the Diabetes Education Program.
- Instruct initial diabetes education clients through classes or individual sessions.
Bariatric Responsibilities
- Able to correctly schedule client for appointment using Cerner Scheduling.
- Provides Pre-surgery and Post-surgery nutritional assessments for clients in the Center for Weight Loss Surgery.
- Develops nutritional care plans and provides nutrition education for individual clients seen in the center.
- Professionally communicates pertinent information to other health care professionals in the Center; discusses individual patient nutrition care needs with health team members; educates health team on nutrition-related topics.
Minimum Requirements
- B.S. or advanced degree in Dietetics.
- . Must be a registered dietitian with the State of Ohio.
- Clinical Nutrition Experience preferred.
