Purpose and Scope

Provides nutritional services for the facility’s in-center and home patients to maximize nutritional status and improve clinical outcomes. Acts as a member of the interdisciplinary healthcare team, supporting the FMCNA commitment to quality improvement activities and actively participating in process enhancements to help patients achieve their individualized goals as determined by their physicians.

Principal Duties and Responsibilities
  1. Assess patient’s knowledge of diet and kidney disease; provide education tailored to learning needs regarding diet, disease impact, and treatment.
  2. Provide detailed, patient-specific education on nutritional status, lab parameters, and fluid management to patient, family, and caregivers.
  3. Complete nutritional assessments and participate in interdisciplinary assessments.
  4. Develop individualized care plans in collaboration with the team, adhering to federal and state guidelines.
  5. Calculate diet prescriptions according to standards of practice.
  6. Maintain accurate documentation and complete progress notes for all patients.
  7. Offer ongoing nutrition assessments and counseling to help patients achieve and maintain effective nutritional status.
  8. Identify malnutrition risks, review lab results regularly, and work with the team to implement intervention plans.
  9. Monitor adherence to nutrition therapy, adjust as needed, and document outcomes and barriers.
  10. Collaborate on recommendations for mineral metabolism, bone disease, anemia, and other nutritional concerns.
  11. Follow FMCNA policies, participate in quality improvement programs, and communicate lab results and treatment plans effectively.
  12. Coordinate with team members for patient education and referrals to physicians for nutritional supplements.
  13. Review nutrition-related labs, counsel patients, and recommend treatment changes as appropriate.
  14. Participate in staff meetings, and consult with corporate dietitians as needed.
  15. Maintain professional knowledge through continuing education and certifications.
  16. Ensure safety and cleanliness in work areas and perform other duties as assigned.
Physical Demands and Working Conditions
  • Work environment typical of healthcare settings, with potential exposure to infectious materials.
  • Requires travel between facilities and participation in meetings.
Supervision

None

Education and Credentials
  • Registered Dietitian certification from the Commission on Dietetic Registration.
  • Board Certified Specialist in Renal Nutrition encouraged.
  • Current state licensure if applicable.
Experience and Skills
  • Minimum 1-year professional experience as a Registered Dietitian, preferably in renal care.
  • Leadership, teaching, organizational, communication, and team skills.
  • Ability to work independently with basic computer skills.
