Job summary

Atrium Health Stanly Diabetes Clinic is seeking a Diabetes Educator to deliver patient-centered education and support for individuals living with diabetes.

Responsibilities include assessing educational needs, developing personalized care plans, teaching lifestyle changes, medication management, glucose monitoring, nutrition guidance, and coordinating with a multidisciplinary team to improve health outcomes. Eligibility requires RN or Dietitian license; CDCES preferred.

Qualifications

• Current RN or Dietitian license in the applicable state; CDCES preferred.
• If not certified, must obtain CDCES within 2–3 years.
• Degree in Nursing or Dietetics meeting CDCES eligibility requirements.

Licensure, Registration, And/or Certification Required

Current license or certification to practice as a Registered Nurse (RN) or RegisteredDietitian, inthe State ofapplicable state. Certified Diabetes Care and Education Specialist (CDCES) preferred. If not certified, you must obtain certification within two yearspreferredbut not to exceed 3 years.

Education Required

Degree in Nursing, Dietetics, or other related disciplines that meet the certification eligibility requirements for CDCES. Ifnursingdegree, BSN preferred. Master's degree in DieteticsrequiredifDieticianlicenseobtainedafter 01/01/2024.

Experience Required

2 years of patient-oriented educational experience preferred with a broad range of knowledge on diabetes for age-appropriate population and its interrelation to co-existing health conditions.

The Diabetes Educator plays a critical role in providing high-quality education and support to individuals living with diabetes. Key responsibilities include assessing patients' educational needs, developing personalized care plans, providing instruction on lifestyle changes, medication management, blood glucose monitoring, diabetes technology education, nutritional guidance, and offering emotional support. The educator will work closely with a multidisciplinary healthcare team, including physicians, nurses, dietitians, and other specialists, to deliver a holistic, patient-centered approach. Serves as a health resource for the community by participating in community events and/or support groups.
