Lakeshore Talent is in search of a compassionate and knowledgeable Dietitian to join a mission-driven healthcare team in San Bernardino, CA. Location: San Bernardino, CA

Schedule: Monday – Friday, 8:00 AM – 4:30 PM

Pay Rate: $68k - $86k (depending on experience)

Position Overview:

The Dietitian will be responsible for completing comprehensive nutritional assessments, developing and implementing care plans, and collaborating with an interdisciplinary team to ensure optimal nutritional support for participants. This role involves both participant care and administrative responsibilities.

Key Responsibilities:

  • Develop and implement individualized nutritional Plans of Care and reassess every six months or as participant status changes.
  • Screen nutritional status, access to food, and coordinate services such as Meals on Wheels or grocery assistance.
  • Provide education and dietary counseling to participants and caregivers.
  • Monitor food intake, therapeutic diets, and recommend liquid supplements as needed.
  • Conduct home visits and communicate with caregivers and staff to ensure proper dietary management.
  • Maintain thorough and timely documentation in participant medical records.
  • Actively participate in interdisciplinary team meetings and care planning.
  • Conduct kitchen and equipment sanitation checks.
  • Evaluate menu adequacy and maintain dietary documentation (e.g., supplement sheets, sack lunch lists).
  • Participate in team meetings, trainings, and department development.
  • Provide guidance and training to Dietary Aides.
  • Coordinate with the State Food Program per applicable regulations.

Requirements:

  • Bachelor's degree in food science, nutrition, or dietetics.
  • Registration with the Commission on Dietetic Registration (CDR).
  • Familiarity with current medical terminology and ability to document care accurately.
  • Experience conducting written nutritional assessments.
  • Must meet California state requirements for Dietitians.
