Job summary

DHR Health is seeking a Dietitian for the Liver Transplant Center in Edinburg to provide outpatient nutritional care to transplant patients and donors. The role requires a registered dietitian with RD credentials, a four-year nutrition/dietetics program, and BLS certification.

Summary:

POSITION SUMMARY:

Coordinates plans and conducts programs to work with transplant patients and their families. Serves in the area of outpatient nutritional care, providing counseling services to patients with kidney and liver disease processes throughout the stages of transplant or organ donation.

POSITION EDUCATION/ QUALIFICATIONS :

• Graduate of an approved four (4) year program required
• MS or MPH preferred
• Registered Dietician (RD) with the Commission of Dietetic Registration required
• Basic Life Support (BLS) required
• Strong verbal and written communication skills

JOB KNOWLEDGE/EXPERIENCE :

• Minimum of two (2) years’ clinical dietician experience
• Prefer experience working with transplant and/or dialysis patients
• Strong public speaking skills
• Demonstrates knowledge of infection control practices
• Demonstrates flexibility, is organized and functions well in stressful situations
• Strong assessment and planning skills, group facilitation, and writing skills
• Ability to apply current evidenced-based nutrition recommendations
• Ability to motivate and enthuse others in initiating behavior change
• Ability to work effectively as a team member while also being able to work independently
• Strong listening skills, and attention to detail

Responsibilities:

POSITION RESPONSIBILITES:

• Performs nutritional assessment of both transplant recipients and potential living donors
• Participates in multidisciplinary rounds and Selection Committee
• Educates patients and their families regarding the nutritional and behavioral aspects of their medical conditions
• Develops and/or implements new approaches to care for the diabetic patient
• Documents assessment and plan of care for the patient in the medical record and transplant database
• Reassesses and motivates patient to improve their nutritional health via follow-up appointments, phone calls, correspondence and telemedicine visits
• Provides feedback to multidisciplinary transplant team in a timely fashion
• Develops and implements plan of care for complex patients and those at high risk for complications
• Plans and coordinates educational classes
• Organizes and participates in support groups
• Coordinates various public speaking and media appearances with the marketing staff
• Provides and coordinates patient education programs
• Develops educational materials for use with patients
• Keeps literature and material current
• Evaluates publications available for patient use
• Incorporates knowledge of current clinical trends, concepts and technology in making sound decisions/judgements and resolving patients’ nutritional problems using scientific methods and intervention strategies
• Collaborates with other specialties to ensure interdisciplinary management and continuity of care for both inpatients and outpatients
• Initiates and facilitates patient care conferences and case reviews
• Provides and coordinates discharge planning for transplant patients with other members of the healthcare team
• Produces required reports in a timely manner
• Adheres to DHR Health and Transplant Institute policies and procedures
• Adheres to the National Patient Safety Goals as outlined by JCAHO
• Adheres to regulatory requirements set forth by Organ Procurement and Transplantation Network (OPTN), United Network for Organ Sharing (UNOS) and Centers for Medicare and Medicaid Services (CMS)
