Job summary

Care Resources PACE seeks a full-time Dietitian to join a team of health care professionals. The role offers a hybrid work model, with mostly remote duties and on-site rotation with other Dietitians. Monday–Friday days, serving participants in the Kent and Ionia counties area.

The successful candidate will assess nutritional status, develop care plans, and coordinate with the inter-disciplinary team to meet participants’ needs. Registration with the Academy of Nutrition and Dietetics is required.

Qualifications

• Advanced degree in nutrition or dietetics from an accredited program.
• Current registration with Academy of Nutrition and Dietetics.
• Experience with a frail or elderly population preferred.

Responsibilities

• Screen and assess nutritional status of program participants.
• Use pertinent data to plan and implement appropriate nutrition interventions.
• Communicate interventions to the interdisciplinary team to ensure nutritional needs of participants are met.

Dietitian Responsibilities

• Screen and assess nutritional status of program participants


• Use pertinent data to plan and implement appropriate nutrition interventions


• Communicate interventions to the interdisciplinary team to ensure nutritional needs of participants are met, including those who are considered to be high risk

Qualifications And Experience

• Advanced degree in nutrition or dietetics from an accredited program

• Prior experience working with a frail or elderly population, preferred


• Current driver's license and insurance to be able to travel around Kent and Ionia counties to visit participants at their place of residence
