Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to support our long-term care community in Pembroke, KY for 4 hours per week or 8 hours every other week. This maternity coverage starts mid-October and offers a flexible schedule with competitive pay.

The role includes completing clinical nutrition assessments, documenting with the Nutrition Care Process in the EMR, collaborating with the care plan team, and contributing to MDS - Section K and wound/weight change recommendations.

Qualifications

• Credentialed by the CDR and licensed as a dietitian.
• Strong communication and collaborative skills for interdisciplinary care.
• Client-centered approach with a positive demeanor.

Responsibilities

• Completing clinical nutrition assessments.
• Documenting using the Nutrition Care Process in the EMR system.
• Collaborating with the interdisciplinary care plan team.
• Completing the MDS - Section K.
• Calculating TF.
• Making recommendations for wounds and significant weight changes.
