Nutrition that Works, LLC is seeking a consultant dietitian to join our team serving the long-term care community in Middleburg Heights, OH. The position offers a flexible schedule with hybrid work for 24–40 hours per week.

You will perform clinical nutrition assessments, document using the Nutrition Care Process, collaborate with an interdisciplinary care plan team, and contribute to wound and weight-management recommendations for residents. Credentialing required.

Qualifications

• CDR credentialed dietitian required.
• Dietetic licensure required.
• Positive attitude and client-centered approach.
• Strong communication skills.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Contribute to wound care and weight change recommendations.

Position includes:

• Completing clinical nutrition assessments
• Documenting using the Nutrition Care Process in the EMR system
• Collaborating with the interdisciplinary care plan team
• Completing the MDS - Section K
• Calculating TF
• Making recommendations for wounds and significant wt changes
