Job summary

The Center for Rehabilitation & Nursing at Washington Township in New Jersey is seeking a Part-Time Dietitian to join our team. The role is 24 hours per week with a pay rate of $30.00–$35.00 per hour, on-site.

You will assess residents' nutrition needs, develop care plans in the EMR, educate residents, families and staff, and work with an interdisciplinary team while adhering to CMS and state regulations.

Qualifications

• Registered Dietitian with current licensure in the state.
• Experience in clinical nutrition and care plan development.
• Strong communication and organizational abilities.
• Familiarity with electronic medical records (EMR)

Responsibilities

• Assess clinical nutrition needs and develop a care plan in the EMR.
• Develop, implement, and evaluate individualized nutrition care plans.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams on nutritional health plans.
• Adhere to facility policies and CMS/state/federal regulations.
• Assist in interdisciplinary care conferences.
• Assist dietitian in duties as assigned.

Dietitian Responsibilities

Registered Dietitian

Pay Rate: $30.00-$35.00 per hour

Monday and Friday with Flexible Schedules

Responsibilities

• Assess the Clinical nutrition needs of residents/patients and develop a plan of care within the electronic medical record.
• Develop, Implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends , intake and comorbidities.
• Provide nutrition education and counseling to resident, families and staff.
• Collaborate with interdisciplinary teams to develop a nutritional health plan for the patient's overall wellbeing
• Adhere to facility policy and procedures and to CMS, State and Federal regulations.
• Assist and participate interdisciplinary care conferences.
• Assist dietitian in duties as assigned.
• Qualifications
• Registered by the Commission on Dietitetic and Registration
• Hold a Certification / License in good standing within the state of practice
• Knowledge of the state survey process and compliance with nutrition care regulations
• Strong organization and communication skills
