Nutrition that Works, LLC is seeking a consultant dietitian to join our team for maternity coverage in Lansdale, PA. The role is 16-24 hours per week in a long-term care setting.

Responsibilities include clinical nutrition assessments, documenting via the Nutrition Care Process in the EMR, and collaborating with the care plan team. MDS - Section K, TF calculations, and weight change/wound recommendations are part of the job.

Qualifications

• CDR credentialed and licensed dietitian.
• Strong communication and client-centered approach.
• Experience with clinical nutrition assessments and long-term care settings preferred.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
