Job summary

City of Hope seeks a Dietitian to assess nutritional status and deliver medical nutrition therapy, education, and care plans for patients. Expect M-F 8:00am-4:30PM with one day remote work weekly, integrating nutrition services with telemedicine and hospital programs.

The role requires a Bachelor's RD/RDN, ongoing CEU credentials, and collaboration with physicians, nurses, and dietitians to optimize patient nutrition and outcomes.

Qualifications

• Education: Bachelor’s prepared RD/RDN.

• Continuing education units (CEU) credentials and ongoing knowledge updates.

Responsibilities

• Formulate and implement nutritional care plans based on assessment of patients’ nutritional needs and physician care plans.
• Monitor and evaluate effectiveness of patients care plan and document in medical records.
• Counsel and educate on nutritional support to patients, families, physicians and staff.
• Collaborate with medical staff, nursing, pharmacy, and other stakeholders to plan patient care.
• Monitor foodservice operations to ensure compliance with diet orders and quality standards.
• Integrate nutrition services into hospital programs such as QAPI and infection control.
• Coordinate clinical nutrition and nutrition support including Telemedicine.

The successful candidate:

Responsible for assessing patients’ nutritional status and intervening with appropriate medical nutrition therapy (MNT) and/or education to improve, maintain, or optimize nutritional status.

JOB RESPONSIBLITIES

• Formulate and implement nutritional care plans based on assessment of patients’ nutritional needs, diet restrictions, nutrition support and physician care plans. Each RD will be able to manage a minimum of 12 patients daily.
• Monitor and evaluates effectiveness of patients care plan. Document thoroughly and appropriately all nutrition care information in patient medical record.
• Counsel and educate on nutritional support and nutritional management to patients and their families, physicians, paraprofessionals, the community and potential candidates for hire within the corporation
• Collaborate with medical staff, nursing, pharmacy, and other hospital stakeholders to plan and implement patient care as necessary in meeting the nutritional needs of patients.
• Monitor foodservice operations to ensure compliance with physician diet orders and quality standards.
• Integrate food and dietetic services into the hospital wide QAPI and infection control programs.
• Responsible for clinical nutrition and nutrition support in coordination with the use of Telemedicine and the use of other nutritional services for patient care needs internally and externally as designated by COH.

Education: Bachelor’s prepared RD/RDN

Licensure: Maintenance of dietetic registration

Training: Continuing education units (CEU) credentials and continually update knowledge base pertinent to job performance
