Dietician - PRN

What You Must Have:

• Bachelor’s Degree in nutrition or related subjects, preferred
• Current State Driver’s License, reliable transportation, and ongoing valid auto liability insurance
• Two years of experience as a Dietician in a clinical care setting, hospice or home health preferred
• Ability to establish and maintain effective working relationships with staff, the public, the Board of Directors, and Medical Director
• Understanding of hospice philosophy, focusing on comfort care and maintaining residence-based care as long as possible

What You Will Do:

• Provide direct dietary assessment of patients
• Develop care plans to meet patients’ dietary needs
• Educate patients and caregivers on diet management
• Report changes and document care to ensure continuity
• Provide counseling and education to IDG, nursing staff, and physicians as appropriate
