Job Description

Ovation Communities is seeking a Dietician to assume professional responsibility and accountability for the nutritional health of residents in collaboration with the director of Food Services and Nursing Home Administrator.

Key Responsibilities

• Complete and update dietary assessments to identify residents’ nutritional needs and risks.
• Conduct interviews with residents, families, and representatives to develop individualized care plans that respect preferences and rights.
• Collaborate with the interdisciplinary team to develop, implement, and evaluate care plans.
• Attend care conferences and communicate resident progress, concerns, and responses to interventions.
• Complete discharge summaries in a timely manner.
• Perform nutritional assessments for assisted living residents as needed.

Qualifications
