Job summary

Satellite Healthcare is seeking a Medical Social Worker to educate patients with kidney disease on treatment choices, health, and wellness to enhance quality of life.

You will support patients’ social and emotional well-being, conduct psychosocial assessments, participate in care planning, and help them cope with chronic kidney disease and dialysis in a multidisciplinary team. A passion for compassionate, patient-centered care and strong communication are essential.

Qualifications

• Master's Degree in Social Work from an accredited CSWE institution.
• MSW required. LCSW/ACSW preferred, LCSW preferred (CA). Licensure/LMSW required in states with licensure (TX, TN).

Responsibilities

• Educate patients on treatment choices to improve health outcomes and quality of life.
• Conduct psychosocial assessments and participate in care planning and team reviews of psychosocial needs.

The Medical Social Worker is responsible for supporting the social and emotional well-being of patients at Satellite Healthcare. The Social Worker conducts psychosocial assessments, participates in care planning and team review of the patient's current psychosocial needs, and provides management of services to patients to assist them in coping with issues associated with chronic kidney disease and dialysis treatments.

Experience

Preferable experience in other health and human services fields. Medical experience is highly desirable. One (1) year of work experience in a behavioral or healthcare‑related setting, preferred.
