Job summary

Satellite Healthcare is seeking a Medical Social Worker to educate patients with kidney disease on treatment choices and wellness, supporting their social and emotional well-being. You will conduct psychosocial assessments, participate in care planning, and manage services to help patients cope with chronic kidney disease and dialysis.

Ideal candidates have a Master’s in Social Work, MSW required, and LCSW/ACSW licensure with CA preference; one year of healthcare-related experience is preferred.

Qualifications

• Master's Degree in Social Work from an accredited CSWE institution.
• MSW required. LCSW/ACSW required, LCSW preferred (CA). Licensure/LMSW required in states with licensure (TX, TN).

Responsibilities

• Educate patients on the best choices for treatment, health, and wellness to enhance quality of life.
• Conduct psychosocial assessments and participate in care planning and team review of psychosocial needs.
• Provide management of services to assist patients in coping with CKD and dialysis treatments.

The Medical Social Worker utilizes social work expertise and clinical knowledge to educate patients on the best choices for treatment, health, and wellness when living with kidney disease to enhance quality of life. The Medical Social Worker is responsible for supporting the social and emotional well-being of patients at Satellite Healthcare. The Social Worker conducts psychosocial assessments, participates in care planning and team review of the patient's current psychosocial needs, and provides management of services to patients to assist them in coping with issues associated with chronic kidney disease and dialysis treatments.

MINIMUM QUALIFICATIONS

Experience

Preferable experience in other health and human services fields. Medical experience is highly desirable. One (1) year of work experience in a behavioral or healthcare-related setting, preferred.

Education
