Job summary

Brooklyn Gardens Dialysis seeks a social worker to support ESRD/CKD patients and families, helping maintain meaningful lives within illness limits. The position requires an MSW with clinical focus, NYS licensure, and experience with dialysis patients.

You will conduct psycho-social assessments, assist with benefits (Medicare/Medicaid/Blue Cross), and coordinate care with a multidisciplinary team and community resources.

Qualifications

• Master's degree in Social Work with clinical practice specialization.
• Certification from the New York State Education Department.
• Must be licensed to practice social work in NY.
• Experience with dialysis patients is required.

Responsibilities

• Conduct psycho-social assessments and determine a treatment plan compatible with medical care.
• Assist patients with understanding and obtaining Medicare, Medicaid, and other insurances.
• Help patients and families access benefits and community resources.
• Participate in multidisciplinary conferences and care planning.
• Coordinate transfers and liaison with hospitals, dialysis centers, and nursing home partners.

Brooklyn Gardens Dialysis is seeking a social worker!

At Brooklyn Gardens Hemodialysis, we are committed to improving quality of life for patients with end stage renal disease (ESRD) and chronic kidney disease (CKD). Our center is staffed by board-certified nephrologists and caring nursing personnel who are dedicated to your comfort, health, and wellbeing.

Provides social case work and support to the ESRD patient and his/her family so that they maintain a meaningful lifestyle within the limitation of the illness and treatment.

RESPONSIBILITIES

• Conducts psycho-social assessments on each patient and determines social work treatment plan compatible with a total medical treatment plan.
• Responsible for knowledge of the benefits available to patients under Public Law 92-603, Section 2991 for Medicare, as well as Medicaid, Blue Cross and other insurance coverage.
• Assists patients with interpretation of benefits available to these patients and assists patients in obtaining the benefits to which they are entitled.
• Participates in multi disciplinary conferences and long-tern, short-term care plan of the patient.
• Utilization of crises intervention techniques in accordance with assessed individual and/or family needs.
• Uses psychiatric consultation in selected situations.
• Refers to appropriate public and private community agencies and assists patients and their families in the use of them: (i.e.) transportation, public assistance, vocational rehabilitation, etc.
• Liaison between the Dialysis Center, affiliated hospitals, Brooklyn Gardens Nursing Home and other dialysis centers. Collects pertinent information for patients transfer or transient visit to other hospital or dialysis centers.

Requirements

• Must have experience working with dialysis patients
• Master's Degree in Social Work with a specialization in clinical practice from an accredited School of Social Work and Certification from the New York State Education Department.
• Must be Licensed
• Qualified by credentials and experience to meet Federal regulations for Social Workers in ESRD Programs.
