- A Home Therapy nurse trains patients in-home and sometimes in-center for hemodialysis or peritoneal dialysis.
- We offer training in in-center hemodialysis, critical care, and home health, with backgrounds in these areas being advantageous.
- Work Schedule: Monday through Friday, 8 am to 5 pm; flexible schedules may be available based on patient caseload.

The Home Therapy Registered Nurse provides patient training and ongoing support for home dialysis patients, ensuring quality care and superior customer service.
Essential Duties and Responsibilities
- Comply with all relevant laws, regulations, and practice acts related to home dialysis and patient care.
- Train patients and care partners in self-care for Peritoneal Dialysis or Hemodialysis, meeting federal and state RN requirements.
- Conduct home visits to assess the patient's environment and performance, initially and annually, with more frequent assessments as needed.
- Communicate assessment results to physicians and team members.
- Document all nursing services accurately in the Electronic Medical Record, including training, interactions, and phone conversations.
- Participate in infection control activities and monitor emergency procedures.
- Ensure adequate supply inventory and timely ordering to prevent issues.
- Use data and critical thinking to improve patient outcomes and participate in quality improvement activities.
- Obtain and communicate lab results, administer medications, and perform on-call nursing services as needed.
- Maintain flexibility in staffing locations and hours to meet program needs.
- Regular attendance is required.
**Home Hemodialysis Specific Responsibilities:**
- Coordinate home environment evaluations with technical services.
- Ensure water sampling and culture testing are completed per policy.
**Qualifications:**
- Current RN license in applicable state, in good standing.
- At least 12 months of clinical nursing experience, with 3 months in the specific dialysis modality (6 months in states with stricter requirements like Texas).
- Preferred: Experience in dialysis care.
- CPR certification prior to patient care.
- Ability to distinguish all primary colors.
- Complete Home Therapy Nurse Orientation and maintain annual competency.
- Proficient in English communication and basic computer skills, with all USRC clinical applications mastered within 90 days.
