Job summary

Baylor Scott & White Health is seeking an RN Patient Navigator for our Outpatient Dialysis program in Temple, TX. This role coordinates patient care, educates patients about dialysis modalities, and collaborates with the care team to improve outcomes.

The RN will conduct patient education, schedule tests, track progress, and participate in quality initiatives while maintaining up-to-date nursing records and supporting patient experience goals.

Qualifications

• Bachelor's degree in nursing required.
• RN license in good standing (Texas preferred).
• BLS certification within 30 days of hire.

Responsibilities

• Conduct new patient orientation and dialysis modality education.
• Provide targeted monthly patient education.
• Partner with leadership on quality improvement and patient experience projects.
• Build strong relationships with patients to improve outcomes.
