Job summary

Concerto Renal Services is seeking a Dialysis Nurse to lead and supervise dialysis care in a skilled nursing or long-term care setting. You will provide direct hemodialysis, educate patients on renal health, and collaborate with physicians to coordinate treatment.

You will supervise dialysis technicians, monitor patient responses, adjust plans as needed, and ensure accurate documentation and regulatory compliance. A hands-on, team-oriented approach is essential.

Responsibilities

• Provides direct hemodialysis care, performs assessments, administers medications, and educates patients on renal health and self-care.
• Delegates tasks to dialysis technicians, supervises their performance, and ensures compliance with clinical standards.
• Monitors patient response to dialysis, adjusts treatment plans as needed, and ensures accurate documentation of interventions and outcomes.
• Coordinates care with physicians and the healthcare team, reports critical lab results, and ensures treatment continuity.
• Maintains electronic medical records and ensures timely completion of care plans and safety protocols.
