Job summary

Concerto Renal Services seeks a Dialysis Nurse to supervise dialysis care within our skilled nursing or long-term care facilities. You will lead a clinical team, provide direct patient care, and support renal health education for patients and families.

You will ensure compliance with state and federal regulations, maintain EMRs, and collaborate with physicians to deliver consistent, high-quality dialysis treatment.

Responsibilities

• Provides direct hemodialysis care, assessments, medications, and education on renal health and self-care.
• Delegates tasks to dialysis technicians, supervises their performance, and ensures compliance with clinical standards.
• Monitors patient response to dialysis, adjusts treatment plans, and documents interventions and outcomes.
• Coordinates with physicians and healthcare team members to ensure continuity of treatment and report critical lab results.
• Maintains accurate electronic medical records, care plans, and adherence to safety and regulatory protocols.
