Job summary

UW Health is seeking a Nurse Practitioner or Physician Assistant for a 100% FTE, day-shift role serving patients at the Mile Bluff, Lake Delton and Portage clinics.

You will participate in multidisciplinary rounds, coordinate care with the team, and manage dialysis orders, anemia, BMD, and dialysis access referrals. Strong clinical skills and Wisconsin licensure required; relocation assistance may be available.

Qualifications

• NP requires Master’s or Doctorate in Nursing; PA requires Bachelor’s or Master’s in PA
• NP licensure: Wisconsin NP licensure, RN/APRN with prescriptive authority and board certification
• PA licensure: Wisconsin PA licensure and DEA license eligibility

Responsibilities

• Participate in multidisciplinary rounds and coordinate patient care with the care team
• Write and manage dialysis orders
• Manage anemia and bone mineral density (BMD) in patients
• Assess and refer for dialysis access placement or intervention
• Manage blood pressure and volume status/dry weight
• Review lab results and coordinate transplant or home dialysis referrals
