Job summary

Satellite Healthcare is seeking a Medical Social Worker to educate patients with kidney disease on treatment and wellness options and to support their social and emotional well-being. You will conduct psychosocial assessments, participate in care planning, and help patients cope with chronic kidney disease and dialysis.

You will join a multidisciplinary team, contribute to care coordination, and support growth and patient outcomes in a compassionate, patient-centered environment.

Qualifications

• Experience in health and human services is preferred.
• One year of work in a behavioral or healthcare-related setting is preferred.
• MSW required; CA licensure (LCSW) preferred; LMSW may be considered in licensure states.

Responsibilities

• Educate patients on treatment options and wellness for kidney disease.
• Educate and support patients' social and emotional well-being.
• Conduct psychosocial assessments and participate in care planning.
• Assist with transitioning patients through dialysis and treatment decisions.
