Qualifications

Job Title: Dialysis Care Partner (In-Home Support)

Platform: Herewith – Flexible Assignments with Families & Partner Facilities

Location: Multiple Locations (Assignments Vary)

Purpose and Scope:
As a Dialysis Care Partner on the Herewith platform, you will provide non-medical, in-home support to individuals undergoing dialysis treatment. Working collaboratively with families and in coordination with clinical providers, your role is to support patient comfort, safety, and post-treatment recovery. You'll also play a key part in promoting independence, reducing hospitalizations, and ensuring smooth care transitions.

Key Responsibilities:
Patient Support & Education:

  • Warmly welcome and support dialysis clients with professionalism and empathy.
  • Assist with patient education on aftercare routines, including hydration, rest, symptom awareness, and nutrition.
  • Support transitional care by reinforcing knowledge about home dialysis options and lifestyle adjustments.
Daily Living Assistance:
  • Assist with mobility, hygiene, dressing, and personal care as needed.
  • Monitor and report signs of post-treatment fatigue, low blood pressure, or discomfort to the designated family or clinical contact.
  • Perform light housekeeping in treatment areas and prepare meals following renal dietary restrictions.
  • Provide medication reminders (no administration) and assist with tracking.
Monitoring & Documentation:
  • Record basic health data (blood pressure, weight, temperature) in compliance with local regulations.
  • Observe and report physical, emotional, or treatment response.
  • Document visits and observations through the Herewith platform.
Infection Control & Safety:
  • Maintain a clean and safe environment following infection-prevention protocols.
  • Educate families on hand hygiene and home cleanliness after treatment.
  • Ensure proper disposal of non-clinical waste and clean high-contact surfaces.
Transitional & Home Dialysis Support:
  • Help new patients understand home-based dialysis routines and equipment (non-clinical).
  • Assist with appointment tracking, follow-ups reminders, and basic supply coordination.
  • Reinforce understanding of care plans as outlined by the clinical team.
Qualifications:
  • High school diploma or equivalent required.
  • Experience supporting dialysis patients (clinic or home settings) preferred.
  • Basic knowledge of renal disease and dialysis is a plus.
  • CPR/First Aid certification preferred.
  • CNA, PCT, or HHA certification is welcomed but not required for non-clinical care.
  • Ability to document and communicate clearly using the Herewith mobile app.
Physical Requirements:
  • Able to stand, bend, lift (up to 30 lbs), and support safe ambulation.
  • Comfortable traveling to client homes (assignments are local and selected via the app).
  • Must pass a background check and onboarding process through Herewith.
