Qualifications

• Graduate of an accredited Medical School (MD or DO).
• Active and unrestricted medical license in Kentucky (Ohio, if applicable).
• Board-certified or board-eligible, if applicable.

Responsibilities

• Diagnose, treat, and manage patient conditions in collaboration with a multi-disciplinary team.
