Cook County Health is seeking a diabetes program leader to supervise a team of certified diabetes educators, nurses, and dietitians. You will clinically assess diabetes patients, develop care plans, and coordinate county-wide nutrition education initiatives.

The role includes budget planning, staff training, ADA-aligned care, travel to Cook County sites, and active participation in ADA programs to advance diabetes management across the network.

Qualifications

  • Experience supervising diabetes care teams.
  • Clinical assessment of diabetes patients and care planning.
  • Budget planning and coordination for county-wide programs.
  • Develop nutrition education materials.
  • Coordinate trainings and orientation for staff.
  • Adhere to ADA standards of care.

Responsibilities

  • Supervises staff of certified diabetes educators including nurses, and dietitians.
  • Clinically assesses patients with diabetes including diet impact; identifies care plans taking into consideration age specific needs of patients.
  • Assists in the development of the fiscal budget. Coordinates the planning of all County-wide Network Diabetes Program activities.
  • Plans and develops nutrition education materials for patients.
  • Trains and supervises staff including nurse, pharmacist, and other health promoters regarding nutrition management of patients.
  • Provides in-service training and orientation and participates in activities in conjunction with the ADA recognition programs
  • Travels to CCH work sites throughout Cook County in the course of duties.
  • Actively participates in the American Diabetes Association (ADA) programs.
