Qualifications

• Two or more years of experience as a certified diabetes educator.
• Associate degree or higher required.
• Two years of college with a major in Health Education, Public Health, Nursing, or Nutrition.

Responsabilités

• Lead Education Sessions: Teach structured classes on genetics and family health history, healthy eating, physical activity, weight management, and stress reduction.
• Deliver Curriculum: Facilitate the approved diabetes prevention curriculum with fidelity to program guidelines.
• Assess Risk: Evaluate participant health behaviors and track weight loss and activity logs.
• Provide Counseling: Offer coaching to help participants set and reach health goals.
• Engage the Community: Partner with local organizations and host outreach events to recruit eligible participants with prediabetes.
• Manage Data: Record attendance, retention rates, and progress metrics for reporting and grant compliance.
