Job summary

AdventHealth in Merriam, KS is seeking an experienced RN diabetes educator to assess, plan, and teach diabetes self‑management. You will work with diverse clients, develop individualized education plans, and coordinate with a multidisciplinary team to improve outcomes.

The role requires an RN license with BLS; a CDE is preferred. A multi‑patient caseload and ongoing education are expected in a full‑time day shift.

Qualifications

• Registered Nurse (RN) required and BLS certification.
• Associate degree in Nursing required; Bachelor's preferred.
• 1+ years RN experience; CDE preferred.

Responsibilities

• Complete comprehensive client assessments and medical history for diabetes care.
• Develop and implement education plans with learning objectives.
• Teach clients in group and individual settings using multiple teaching methods.
• Evaluate progress toward learning objectives and behavior change goals.
• Coach clients on behavior changes for diabetes self‑management.
• Document encounters accurately and timely; share progress with physicians.
• Collaborate with multidisciplinary team to meet client needs.
• Manage a multi‑client caseload while maintaining confidentiality.
• Assess glycemic control via labs and self‑monitoring, adjust as needed.
• Maintain CDE and BLS certifications; pursue ongoing education.
• Promote the education program to staff, physicians, and community; support QA.
