Job summary

Qualifications

• Minimum of three years RN experience or equivalent combined experience
• Bachelor’s degree in nursing (BSN) required
• Effective verbal and written communication skills required
• Ability to prioritize and make independent clinical judgments
• Experience with basic patient care equipment and policies
• Ability to work collaboratively with an interdisciplinary team
• Licensed RN in Nebraska or a compact state
• BLS certification from an AHA-endorsed program

Responsibilities

• Coordinate a patient care delivery system for a designated group of patients across ambulatory settings
• Provide targeted interventions to avoid hospitalization and ER visits
• Assist complex patients in transitions of care across multiple settings
