Qualifications

• Graduate from an accredited Nursing Program with RN/LPN licensure.
• CDCES certification is required.
• CPR/BLS certifications supported for clinical practice.
• Experience educating diabetes patients in outpatient settings is preferred.
• Strong communication and teaching skills in English.

Responsibilities

• Assess and educate patients with diabetes in individual or group sessions.
• Provide self-management education on SMBG, CGMS, and insulin/pump basics.
• Collaborate with care team to coordinate preventive and chronic care.
• Perform telephonic triage and patient education, ensuring PHI protection.
• Lead and participate in quality improvement activities and staff education.
