Job summary

Wellstar Health System in the United States is seeking a Diabetes Educator/Dietitian to coordinate diabetes self-management and nutritional care across all phases of care. You will assess biological, emotional, developmental, and education needs and deliver patient-centered education to individuals and families within the DSMES framework.

As part of the diabetes care team, you will educate on nutrition interventions, monitor outcomes, and document care in the EHR.

Qualifications

• Clinical assessment and education in diabetes self-management.
• Knowledge of DSMES standards and nutrition care process.
• Experience with diabetes technology and data interpretation.
• Ability to educate patients and families across ages.

Responsibilities

• Coordinate diabetes self-management education and nutritional care.
• Assess patient nutrition needs and develop care plans.
• Document in EHR and track outcomes.
• Provide education on monitoring tools and insulin pumps.
• Collaborate with interdisciplinary team.
• Participate in quality improvement and LEAN Six Sigma.
