The Diabetes Educator/Dietitian at Wellstar Health System coordinates diabetes self-management education and nutritional care across all care phases. • Coordinate diabetes self-management education and nutrition care across all phases.
• Educate patients and families about diabetes management and nutrition.
• Assess biological, emotional, nutritional needs to inform care.
• Document encounters in the EHR per policy.
• Participate in quality improvement and LEAN Six Sigma initiatives.

Responsabilidades

• Deliver DSMES services in outpatient or ambulatory settings.
• Translate data from glucose monitoring tools into care plans.
• Provide technology-enabled diabetes education to patients and staff.
• Assist with prior authorizations for patient equipment and supplies.
