Qualifications

• Bachelor’s degree in Nursing, Dietetics, or related field.
• Valid Maryland licenses as Dietitian, Nurse, or Nurse Practitioner.
• Certified Diabetes Educator certification preferred or eligible; certification must be obtained within 1.5 years if not held.
• Strong communication, organizational skills, and basic computer proficiency, including Excel.

Responsibilities

• Develops patient-centered diabetes care plans in collaboration with healthcare professionals.
• Provides comprehensive diabetes self-management education and medical nutrition therapy through individual and group sessions.
• Completes assessments, sets educational goals, and monitors patient progress via in-person, phone, or virtual visits until goals are achieved or transfer occurs.
• Maintains accurate records, documents care and billing accordingly, and partners with diabetes institutes to record outcomes.
• Participates in multidisciplinary teams, quality improvement initiatives, and community outreach.

• Develops patient-centered diabetes care plans in collaboration with healthcare professionals such as nurse practitioners, primary care providers, and endocrinologists.
• Provides comprehensive diabetes self-management education and medical nutrition therapy through individual and group sessions, tailoring teaching methods to patients' learning styles.
• Completes assessments, sets educational goals, and monitors patient progress via in-person, phone, or virtual visits until program goals are achieved or patient transfer occurs.
• Maintains accurate records, documents care and billing appropriately, and partners with diabetes institutes to record outcomes.
• Participates in multidisciplinary teams, quality improvement initiatives, community outreach, and serves as an educational resource within the practice.
