Job summary

UPMC is seeking a Diabetes Educator to provide diabetes education to patients referred to the Diabetes Education Network in outpatient, inpatient or community settings, and to support other teams within Endocrinology.

Travel to Erie, PA and Johnstown, PA may be required. Responsibilities include teaching self-management, supporting ADA recognition for educational programs, documenting encounters, and coordinating care with clinicians for optimal glycemic control.

Qualifications

• Bachelor's degree in nursing or related health care field preferred.
• 2 years clinical experience required.

Responsibilities

• Educate patients and families on diabetes self-management according to national standards.
• Assist in achieving/maintaining ADA Recognition for Health System's educational programs in outpatient, inpatient or community settings.
• Instruct patients and family members/significant others in Healthy lifestyles, Blood glucose testing, Medication management, Risk reduction, Insulin administration, Insulin pumps.
• Assess educational and management needs and create a care plan in various settings.
• Manage ongoing education/care for patients, consulting with physicians as appropriate and monitoring glycemic control.
• Provide training and updates on diabetes education to staff.
• Document all patient interactions and ensure appropriate billing for diabetes education services.
• Adjust medication/insulin doses according to approved protocols when appropriate.

A Diabetes Educator provides diabetes education to patients referred to the Diabetes Education Network in outpatient, inpatient or community setting. Provides support to the other functions in Endocrinology i.e., specialized procedures.

Travel may be required to Erie, PA & Johnstown PA.

Responsibilities:

• Provides education according to the National Standards for Diabetes Self-Management Education in association with all relevant aspects of health care for the person with diabetes.
• Participates in achieving/maintaining American Diabetes Association Recognition for Health System's educational programs in an outpatient, inpatient or community setting.
• Instructs patients and family members/significant others in the implementation of Healthy lifestyles, Blood glucose testing, Medication management, Risk reduction, Insulin administration, Insulin pumps.
• Assesses diabetes patients' educational and management needs and creates a care plan in an outpatient, inpatient or community setting.
• Manages on-going education/care for patients (consulting with physician as appropriate) by answering questions, reviewing home glucose monitoring records, reviewing diabetes medications accordingly and monitoring glycemic control.
• Educators are expected to provide training and updates on diabetes education to staff.
• Accurately documents all patient interactions and ensures appropriate and complete billing for diabetes education services.
• Adjusts medication/insulin doses according to established approved protocols when appropriate.

Licensure, Certifications, and Clearances:

• ACSM Certified Exercise Physiologist (ASCM-CEP) OR Doctor of Medicine (MD) OR Doctor of Osteopathic Medicine (DO) OR Doctor of Podiatric Medicine OR Licensed Master Social Work (LMSW) OR Master Certified Health Education Specialist (MCHES) OR Occupational Therapist (OT) OR Optometrist OR Pharmacist OR Physical Therapist (PT) OR Physician Assistant Certified (NCCPA) OR Psychologist OR Registered Dietitian (RD) OR Registered Dietitian Nutritionist (RDN) OR Registered Nurse (RN)
• Act 31 / Act 33 / Act 34 / Act 73 with renewal
• Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
