Job summary

Holy Cross Health in Maryland is seeking a part-time Diabetes Educator to plan, implement, and evaluate education for patients and the broader community. The role mentors staff and coordinates with Community Health programs to improve diabetes self-management and reduce hospital utilization.

Ideal candidates will have a health education background, CPR certification, and relevant clinical experience. Bilingual English/Spanish is preferred, with a focus on confidentiality and quality of care.

Qualifications

• Bachelor’s degree in health-education related field.
• Master’s Degree in clinical specialty area or related field preferred.
• CPR certified within probationary period.
• RN, RD or pharmacist with training and minimum of 2 years’ experience pertinent to Diabetes Self- Management Education and Support.
• Certified Diabetes Care Educator Specialist (CDCES) or Board- Certified Advance Diabetes Management (BC-ADM) certification preferred.

Responsibilities

• Plan, implement, and evaluate education for patients and community.
• Mentor and educate staff within Diabetes Education Programs.
• Support community health goals and partnerships to reduce hospital utilization.
• Contribute to mission alignment with Trinity Health and Holy Cross Health.
